Job Description

Job Summary: Face to Face Medical Management is seeking a highly qualified and experienced Nurse Practitioner to join our team. The successful candidate will be responsible for providing excellent medical care to our patients in collaboration with the Interdisciplinary Group. The Nurse Practitioner will assess, diagnose, and treat acute and chronic diseases, and promote positive health behaviors through education and counseling. Responsibilities The Nurse Practitioner will work in conjunction with the Medical Director, Practice Manager, Director of Nursing, and the members of the Interdisciplinary Group to: Obtain medical/health histories and perform physical examinations Diagnose, treat, and monitor acute and chronic diseases in accordance with the plan of care Order, perform and interpret diagnostic studies Identify, implement and evaluate appropriate individual plans of care, including pharmacologic interventions within the guidelines of the ARNP Practice Act of the state in which the medical practice is licensed Promote positive health behaviors and self-care skills through education and counseling Act as a care manager of the patient by assessing and modifying the plan of the care with the Interdisciplinary team Provide comprehensive functional assessments that include activities of daily living, instrumental activities of daily living, economic, environmental, mental status, and social assessments Provide care in multiple sites that include but are not limited to the patient’s home, assisted living facilities, group homes, hospital, and nursing homes Provide accurate and appropriate documentation of patient/family services as specified in policies and submits this documentation within 48 hours of when note is closed Participate in the team responsibility of providing 24 hour a day/7 day a week patient care Act as a liaison between the practice and other professionals including physicians, hospitals, nursing homes, resource agencies, and the medical community Demonstrate knowledge of Face to Face Medical Management policies and procedures and pertinent State and Federal Rules and Regulations Recognize, reinforce and participate in the Interdisciplinary Group Meetings Support the mission of Face to Face Medical Management through formal and informal education Participate in continuing professional development and maintenance of clinical skills Act as a health care resource, interdisciplinary consultant, and patient advocate Participate in CQI activities Assist with employee health activities Responsible for monthly and as needed follow up with Outreach Program patients Provides after-hours on-call services via telephone, as assigned Participate in Bi-weekly IDG rotation with Medical Director Assess declining patients & when appropriate refer to hospice Works in collaboration with Infinity Hospice Care with transitioning hospice patients Visit high-needs DCE patients – adjust frequency and increase visits as needed Perform telehealth visits when appropriate If needed, perform blood draws and dressing changes if applicable Perform other duties as assigned Qualifications Current license as an Advanced Registered Nurse Practitioner in the state in which the medical practice is licensed. Bachelor’s Degree in Nursing. Minimum of two (2) years of nursing experience. Minimum of two (2) years experience as an ARNP with experience in gerontology experience with pain and symptom management is preferred. Must have reliable transportation, which will enable the ARNP to perform tasks and responsibilities in a timely and appropriate manner. Must provide proof of automobile insurance. Strong verbal and written communication skills. Strong assessment skills.
